The RealAge Test is your first step, as it helps you understand which of your good and bad habits are impacting your health. From there, Sharecare provides you with content and programs to help you improve your overall health and obtain a younger RealAge. It takes about 10 minutes to complete the RealAge Test. The answers you provide will not be shared with your employer.

Q: WHAT IS RealAge?

A. What’s RealAge? It’s how old your body really thinks you are based on your health and health habits, good and bad. To find out, take the RealAge Test — it’s based on lifestyle, genetics, and medical history. Up to 70% of overall health depends on how well you take care of yourself, so you have a lot of control over how young you feel!

Q: WHY SHOULD I PARTICIPATE?

A. Your well-being is important. With the right information, you can be better prepared to address any health risks that could possibly affect the quality of your life. FAQs – Biometrics

Q: HOW CAN I PARTICIPATE IN A BIOMETRIC SCREENING?

A. There are three ways to participate in a screening:

Option 1:  AT A BE WELL SHBP SPONSORED ONSITE BIOMETRIC SCREENING:

Screenings will be conducted conveniently and privately at authorized SHBP screening locations throughout the state. Be Well SHBP sponsored onsite biometric screenings events will begin at a later date to be announced. You can schedule your screening appointment up to 48 hours before the event date.

Option 2: WORK WITH YOUR PHYSICIAN:

If you prefer, you may also work directly with your Physician. To download the 2021 Physician Screening Form, click here and follow the instructions on the form.

Download your Physician Screening Form here

Option 3: GET SCREENED AT A QUEST DIAGNOSTIC PATIENT SERVICE CENTER (PSC):

There is no need to bring any paperwork; simply log in to Quest’s online scheduling tool and sign up for a PSC screening location near you. For your convenience, same day and next day appointments are available.

Sign Up for, or Schedule, Your Screening Appointment

Q: WHAT HAPPENS DURING THE BE WELL SHBP SPONSORED ONSITE BIOMETRIC SCREENING?

A: During the screening, a health professional will collect measurements, including height and weight. A sample of your blood will be collected to determine your cholesterol and glucose levels, triglycerides and other factors that can lead to lifestyle-related health complications. The screening does not test for illicit drugs, HIV/AIDS or hepatitis.

  • You will get a personalized report highlighting your screening results
  • You get a detailed explanation of what your numbers may mean and the healthy target ranges
  • You will learn about areas for improvement and recommendations if needed

Q: WHY SHOULD I PARTICIPATE?

A. Because some risk factors may have no symptoms, you could be at risk without even knowing it. A biometric screening will help you to understand the current state of your well-being, so you can take the necessary steps to improve it. All SHBP members and spouses covered by non-Medicare Advantage Anthem or UnitedHealthcare who complete a screening are eligible to earn 120 well-being incentive points. You must complete your RealAge Test before well-being incentive points will be awarded. Dependents are not eligible to participate in the biometric screening portion of the Be Well SHBP well-being program and will not be eligible to earn well-being incentive points.

Q: HOW MUCH DOES IT COST?

A. There is no additional cost to SHBP members or covered spouses for participating in a Be Well SHBP sponsored onsite biometric screening, at a Quest Diagnostic Patient Service Center, or for a screening through their physician as long as the screening is completed as part of a preventive visit with an in-network physician and properly coded as preventive. If you have your screening completed at the same time you receive other medical services or treatments (for example, a sick visit), you may have to pay co-insurance/co-pay. Some physicians might charge a fee to fax the Physician Screening Form to Sharecare. You can obtain a copy of your completed form and directly fax it to 615-349-9111 or upload it to Sharecare.

Q: WHO ADMINISTERS THE BE WELL SHBP SPONSORED ONSITE BIOMETRIC SCREENING?

A. The screening is administered by Sharecare. Sharecare protects the confidentiality of your information in compliance with the requirements of the Health Insurance Portability and Accountability Act (HIPAA) of 1996 and applicable state laws.

Q: HOW DO I SIGN UP FOR A BE WELL SHBP SPONSORED ONSITE BIOMETRIC SCREENING?

A: At this time, onsite screenings are not available. We are hoping to offer onsite screenings in 2021 and will provide updates and communications regarding how to schedule your appointment. For now, you can complete your biometric screening through your own physician using the 2021 Physician Screening Form or at a Quest Patient Service Center.

Q: HOW DO I PREPARE FOR THE SCREENING?

A:  Here are some things to keep in mind as you prepare for your screening:

  • Although not required, we recommend fasting for at least nine (9) hours prior to your screening; this includes all food and drinks except for water. Failure to properly fast may result in inaccurate blood test results. (If you have a health condition, such as diabetes or pregnancy, or any other medical concerns, consult a doctor before fasting)
  • Drink plenty of water, which makes it easier to give a sample of your blood
  • Continue taking medication as prescribed the day of your screening*
  • Arrive five minutes early to register and allow about 15 minutes to complete the process

*Follow your doctor’s instructions for taking your medications, including taking them with food if necessary.

Q:  WHAT IF I DON’T SEE MY BIOMETRIC SCREENING RESULTS POSTED IN MY HEALTH PROFILE?

Q: WHAT HAPPENS IF I TRANSFER TO MEDICARE ADVANTAGE THIS YEAR?

A: Remember to redeem well-being incentive points before transferring into a Medicare Advantage plan as points are not automatically redeemed and transferred for Medicare Advantage members.

Any unused well-being incentive credits will remain in your Health Reimbursement Arrangement (HRA), Health Incentive Account (HIA), MyIncentive Account (MIA) for a six month run out period, to allow for prior year’s claims processing.

If you have a balance of 100 credits or more in your HRA, HIA, or MIA after being enrolled in Medicare Advantage for at least six months and are not in a split option, an individual Retiree Reimbursement Account (RRA) will be set up by your Medicare Advantage Plan Administrator, Anthem or UnitedHealthcare.

Credits remain with the active Commercial Plan Option if you have a spouse and/or dependent(s) who is still on that plan. If there are any credits remaining once the last Commercial member moves to Medicare Advantage, the credits will follow that member. Once you move over to a Medicare Advantage Plan Option, you are no longer eligible to earn well-being incentive points with Sharecare.

Q: HOW DO THE WELL-BEING INCENTIVE POINTS REDEEMED AS CREDITS SAVE ME MONEY?

A. Depending on the health plan option you selected, the well-being incentive points you redeem as credits work a little differently. But all well-being incentive credits can be used to help offset eligible medical and pharmacy expenses.

EARN

Complete the healthy actions between January 1, 202q and November 30, 202q and earn up to 480 well-being incentive points (up to 960 per household). You and your covered spouse can redeem points earned in increments of 120 up to 480.

  DEPOSIT

Once you redeem your points for credits, SHBP will  deposit your well-being incentive credits  into the account associated with your health plan within 30 days:

  • Anthem HRA
  • Anthem MyIncentive Account (MIA)
  • UnitedHealthcare Health Incentive Account (HIA).
  SAVE

Your well-being incentive credits work like funds in that account helping you to offset eligible medical and pharmacy expenses throughout the year like co-pays, co-insurance and deductibles.

Click here to see how they work with your health plan.

Q: WHEN WILL I RECEIVE MY CREDITS ONCE I REDEEM MY POINTS?

A. Once you redeem your well-being incentive points for credits, Sharecare notifies your Health Plan provider. Within 30 days, your well-being incentive credits will be available in your Anthem MyIncentive Account (MIA), Anthem Health Reimbursement Arrangement (HRA) or your UnitedHealthcare Health Incentive Account (HIA). Contact your health plan directly to confirm the balance available for use.

Q: WHO DO I CALL IF I HAVE A QUESTION ABOUT POINTS?

A. For questions about how to EARN well-being incentive points, call Sharecare at 888-616-6411. For questions about the amount of well-being incentive credits available for use in your Anthem MyIncentive Account (MIA), Anthem Health Reimbursement Arrangement (HRA) or your UnitedHealthcare Health Incentive Account (HIA), contact your Health Plan directly.

Q: CAN MY SPOUSE EARN POINTS TOO?

A. All SHBP covered spouses are eligible to earn up to 480 well-being incentive points. That is a family total of up to 960 well-being incentive points. Note: Covered dependents are not eligible to earn incentives.

Q: WHAT IS A “GREEN DAY”?

A. A “green day” is a measure Sharecare uses to help you quantify achievements you’ve made toward improving your health, essentially empowering you to improve your RealAge in real-time by tracking the most critical health factors that impact your RealAge: stress, activity, sleep, relationships, weight, blood pressure, blood glucose, cholesterol, tobacco use, drinking, diet, medications, and fitness. With each key health factor rated on the five-point color scale from green to red, your goal is to be “in the green” for 8 of the trackers per calendar day to earn what we call “a green day.” And each time you hit 60 green days within a 90-day period, you are “living in the green,” which prompts an automatic recalculation of your RealAge to see how much you’ve improved your health during that time.

Q: WHAT IS THE GREEN DAY TRACKER?

A. Sharecare Green Day Tracker (GDT) includes daily trackers, which are core to the RealAge calculation.  The trackers include, steps, sleep, stress, relationships, blood pressure, weight, tobacco exposure, cholesterol, alcohol, fitness and health, diet, medications, and blood glucose.  In order to accomplish a Green Day, you will need to enter data for 8 trackers within the green range daily.

Q: HOW DO I KNOW WHICH TRACKERS TO UPDATE?

A. The trackers that you are required to update daily are based on the results of your RealAge Test. The trackers that appear in green are automatically populated from the results of your RealAge Test or from your health profile. The trackers that appear in white must be updated. Your health status may impact how often you are asked to confirm certain data. For example, if you have conditions or risks related to blood pressure or blood glucose, you would be prompted for a daily entry to that tracker, or it will be pre-set to green if your health status does not require daily monitoring of blood pressure or cholesterol. If you indicated that you don’t use tobacco or drink, the tracker will be pre-set to green and you will be prompted for reconfirmation on a quarterly basis or less.

Q: WHAT IS NEEDED TO ACHIEVE A GREEN DAY?

A. Each factor is measured on a scale derived from a green to red index. The scale is color-coded from green (best) to red (worst) as a way for you to more easily understand your measures at a glance. See below.

Q: CAN I CONNECT MY FITBIT OR OTHER WEARABLE DEVICES?

A. Yes. You can now enable Fitbit thru the Tracker settings screen. You will need to set up the connection to Fitbit using your Fitbit credentials to sync the device. If the Fitbit device is not synced, we are not able to receive the data in the Sharecare app. To optimize steps tracking and make sure you get every step possible, Sharecare will use a smart algorithm to collect steps data. The steps data will display a total number with the attributed source i.e., Fitbit, Apple Health to make it clear to you what number and source is being used for the tracking. Once Fitbit is connected, you may decide to disconnect and no longer sync the Fitbit data by disconnecting the Fitbit.

Q: ARE THERE GREEN DAY TRACKERS THAT UPDATE AUTOMATICALLY?

A. Yes, by automatically using the mobile phone sensors through the app. For Android devices, this tracking is available for sleep, steps, stress and relationships. For Apple devices, this tracking is available for sleep and steps.  The instructions in the trackers will show you how to do this.  As you engage with the app, you will be prompted to set up the automatic tracking.  You may also set this up within each individual tracker by going to the wheel cog at the right corner of the app.

Q: What if I am unable to get “in the green” within the Green Day Tracker?

A. To earn a green day, you will need to enter 8 trackers within the green range daily. If you find you are unable to consistently earn green days on the website or through the app, you are encouraged to engage in telephonic coaching as an alternative to tracking, to help improve your lifestyle and work towards earning green days.
