SHBP Exceptions for Participation in 2024 Well-Being Program
We understand there may be circumstances that can prevent you or your spouse from being able to participant in the 2024 well-being program. For these situations, exceptions apply to you or your covered spouse who are unable to participate in the 2024 Well-Being Program due to medical conditions or circumstances beyond your control. These situations include but are not limited to the following:
Critical Illness
Dementia
Mentally Incapacitated
Hospice
Long Term Hospital Stay
Active Military Duty
If you think you may be unable to complete the 2024 Well-Being Activities, you may qualify to meet the wellness requirement in a different way. Please contact Sharecare at 888-616-6411 and we will work with you (and, if you wish, with your doctor) to find a wellness program alternative that is right for you considering your health status.
We will need documentation from your physician for medical exceptions and the reason why the exception is needed. And, we need documentation for any other exception. Please complete the Appeal Level 1 form to apply for this exception.
Pregnancy – We need a note from your physician stating that you are unable to participate in the biometric screenings. Members and/or spouses can still complete the RealAge® Test and participate in coaching.
