A new way to earn your 480 well-being incentive points in 2021 by receiving the COVID-19 vaccination!

If you are an eligible State Health Benefit Plan member or a covered spouse, SHBP has added a new promotion which adds the COVID-19 vaccination as an activity worth the full 480 well-being incentive points which eligible members can redeem for 480 well-being incentive credits or a $150 Visa® Reward card.  This wellness program addition will also end November 30th.

Eligible vaccines include one dose of Johnson & Johnson (Janssen), or two doses of Pfizer-BioNTech or Moderna.


Follow the steps below to earn your well-being incentive points:

  1. Register or log in to the Be Well SHBP program 
  2. Complete the RealAge Test if you have not done so in 2021 
  3. Select Achieve > Rewards
  4. Click 2021 State Health Benefit Plan Rewards Program
  5. Select COVID-19 Vaccination – Self Attest or COVID-19 Vaccination Claim
    If selecting Self Attest, click Update Immunizations, then click Add New and enter COVID in the search bar. Next select the vaccine you received and enter the date. If you received a 2nd shot, repeat this by clicking Add New again. When selecting your vaccine dose(s), please remember to choose the option with “1st Dose” and “2nd Dose” (for Pfizer-BioNTech or Moderna).
    • If selecting Vaccination Claim, click View your Results. (If you do not see a claim for your vaccination, please proceed with the self-attest option.)

Once you have completed the above steps, you have earned your well-being incentive points for the year, which you’ll see in the Rewards section. You can redeem your points for a $150 Visa® Reward Card or 480 well-being incentive credits that can be used towards medical and pharmacy expenses.

Important Note: You can earn a maximum of 480 well-being incentive points in 2021. If you have already received all or part of your well-being incentive points, you will only receive well-being incentive points up to the yearly maximum of 480. The same is true for a covered spouse. Dependents are not eligible for incentives.
